2. Of our 7 locations throughout Florida, select the location most convenient to you.
3. Click Hold My Spot in Line.
4. Complete the short information to help preregister you and help expedite your visit. Information includes patient’s reason for visit, name and date of birth.
5. Select a convenient time for your visit and check the box to receive alerts about your visit time.
6. Click Confirm Me.
7. Once you arrive at the clinic, please check in at the front desk kiosk.
